Student-Athlete Forms

To: Student-Athletes, Parents and/or Guardian

The following forms are needed to complete the student-athletes medical file for the 2007-2008 school year.

These forms need to be signed by the parent and/or guardian and student-athlete in the appropriate places and returned to the university before July 25th. 2008.

If the forms are not completed and returned by the above date, it will delay the student-athlete from participating in intercollegiate athletics at Eastern New Mexico University.

It is very important that we receive a copy, front and back of the insurance card. If you have no health insurance, please write “no insurance” on the forms that ask for insurance information.
